Holistic Case Management

Here’s how Holistic Case Management works:

The case manager conducts an extensive interview with the participant to gather information about all aspects of the participant’s life so that, together, they can craft a comprehensive family development plan that works on all levels.

The interview will include (but is not limited to) information on:

  • Employment
  • Education
  • Childcare
  • Transportation
  • Housing
  • Health care
  • Income management
  • Nutrition
  • Substance abuse
  • Mental health

The plan you create with the case manager will identify any possible barriers to achieving the agreed-upon goals so you boost your chance of success.

The case manager monitors the participant’s progress from their entry into the program through landing employment, and will continue until the client reaches economic independence (or no longer participates in the program).

Services we offer as part of holistic case management support may include (but are not
limited to):

  • Childcare services
  • Transportation
  • Educational assistance
  • Assistance with energy cost/housing cost.

If necessary, we will refer the participant to educational facilities, life-skills training, and job-search activities that are a match for their employment and life goals.

When two community-service organizations offer different sets of services to low-income individuals and families, it just makes sense to partner and collaborate, so the clients of both organizations have access to the full roster of services. Establishing and strengthening these cooperative linkages with various partners continues to be a key goal for Tallatoona CAP, Inc.
